Registered Nurse (Discharge Planner)
The inpatient Discharge Planner is responsible for assuring that all veterans in the inpatient units receive discharge planning that begins in admission and incorporates a multi-disciplinary approach to facilitate timely discharges. The RN Discharge Planner will also discuss key areas to prevent problems, educate, assess and listen to and honor the patient goals and concerns.
